Military Records Request
This form must be completed by applicants whose files have been selected to begin a background investigation, regardless if whether the applicant has served in the military or not. If you were never in the military, complete Section I, numbers 1-4. Write "did not serve in the military" in box number 5a, Branch of Service. Sign at the bottom in Section III, number 3.
References Form
This form must be completed by applicants whose files have been selected to begin a background investigation.
Valencia Student Consent
